Why one record matters

The common alternative is a quality system that sits beside the hospital's actual operations. Staff do the work, then someone re-enters a version of it into the quality system so there is something to show. The re-entry is the expensive part, and it is also where the record stops matching reality — which is what an experienced assessor is probing for in the interview.
